Glycemic Management (Glucomanager)

Glucomanager is a data management tool that allows member organizations to monitor in-patient blood glucose levels by unit, shift. patient and range. Glucomanager is a web based product that requires minimal maintenance from the hospitals to use.

Summary

Glucomanager is a glucose tracking application that allows for the easy collection, analysis, reporting, and displaying of glucose readings obtained from hospital glucometers. The use of Glucomanager will facilitate the best outcomes for glucose control and can be configured to run data by shift, physician, patient, individual unit, and / or groups of units. Improvement and control of blood sugars will better position hospitals to meet CMS and payer initiatives to reduce wounds, infections, and mortality, thus decreasing length of stay, costs per case, and reportable never events.

Glucomanager was developed by the Performance Improvement Department of VHA Georgia in response to a glucose management benchmarking project with member hospitals. The application is not limited by the number of units or patients, is easy to use on a secure web based network, and does not require additional staff or equipment by the hospital. Glucomanager is currently under a U.S. patent.

Objectives

• Promote best clinical outcomes in glucose control in VHA Georgia member hospitals and others utilizing comparative dashboards;
• Continue to work with IHI, CMS, and other best practice, research, and regulatory agencies to promote the broad reaching clinical aspects of Glucomanager.
