ICD-10 Description of Services

High level and mid-level
Overview of ICD-10 and implications for the hospital and affiliated entities by experienced health information professionals. Annual update of the industry status of ICD-10 with emphasis on where healthcare facilities should be in their implementation process.

Physician/CDI
Overview of ICD-10 and implications for the practice. 4-6 hour one-on-one education for physician champions and clinical documentation improvement specialists utilizing results of clinical documentation improvement assessment.

Detailed ICD-10 Coding Education
Coder bootcamp in various locations throughout GA over several months; conducted by AHIMA certified ICD-10 Trainers. Program will be designed to permit physician office and ambulatory service staff to participate. Participants required to submit de-identified records for lab portions of training.

Coder Skill Assessment
To determine re-education/refresher requirements. Optional feedback and guidance services are available for the individual.

Apprentice program
“Growing” coders for participant facilities by developing a 60+/- week curriculum in collaboration with participating facilities, technical schools and/or others. Each facility will provide apprentice opportunities for participating students. Pursue discounts from AHIMA for their Coding Basics computer-based training program and related texts for facility based educational opportunities.

Coding Pool
Offering contract coding support. Coders will be tested for skill levels, receive ICD-10 education and be routinely monitored for quality. Coding will be conducted remotely through VPN connections to participating facilities; on-site services may be available on a limited basis.

Clinical documentation improvement initiatives
Assist organizations in defining what is needed for the organization. May assist in selection of CDI firm. Define CDI plan for organization. (Also see “Fiscal Guidance.)

Coding Quality Reviews
After ICD-10 go-live, periodic coding quality review services, remedial education as indicated.

Anatomy and Physiology Audio conference
Two half day or 1 all-day A&P refresher educational program led by experienced A&P instructor. Saturday option possible.

Assessment: (assuming ~200-250 bed facility)

Consultant guided
Supply data collection survey tool, template for action plan, and walk through key readiness components with participating team members; participating organization captures findings and develops initial Action Plan.
  • Consultant reviews and provides input to Action Plan, as applicable.

Consultant driven
Consultant surveys management team members, compiles data, defines individuals to interview, conducts interviews, captures findings and develops initial Action Plan.

Follow-Up and Project Management

  • Consultant follows up at every other month intervals or more frequently to determine if participant hospital is progressing on schedule, provides guidance as necessary.
  • Consultant utilizes Action Plan, updates and modifies Plan as appropriate, and:
    • Canvasses each vendor and healthplan to determine the vendor’s/healthplan’s readiness plans and upgrade schedule, interface modifications, and schedules testing in collaboration with IT and/or source department.
    • Develop education plans for each category of participant facility staff.
    • Collaborates with decision report on revamping reports to allow for comparative data during cross-over period.
    • Participates in system/application upgrade and/or replacement discussions.
    • Data mines and maps data to assist managed care in assessing contractual modifications.
    • Data mines and maps data to develop documentation education for clinicians and clinical documentation improvement.
    • Canvasses trading/communication partners to determine partner’s knowledge level of ICD-10, provides education as needed, develops plans for data transfer, schedules tests in collaboration with IT and/or source department.
    • Coordinates the updating of encounter forms/superbills as directed.
  • Prepare periodic updates for management and senior leadership.

Fiscal Guidance

Fiscal/Documentation Improvement Support
Analyze top MS-DRGs and identify documentation requirements under the ICD-10 model of coding to serve as clinical documentation improvement program foundation and forecast reimbursement impact.  Evaluation assesses whether same or other MS-DRG is supported by documentation required for ICD-10.
